Following referral from your healthcare professional, one of our specialist nurses may arrange an assessment either in your home, in a clinic setting, or virtually where appropriate.

The assessment provides an opportunity to discuss your individual circumstances, current treatment and any challenges you may be experiencing. By understanding your needs, we can help identify the most appropriate support and develop a personalised plan designed around you.

Our goal is to help you achieve the best possible outcomes while feeling informed, comfortable and supported.
